Lymphome malin B de type centrofolliculaire, lésion initiale.

Diagnosis:

 

 

 

 

 

 

 

 

 

 

Cutaneous follicle centre lymphoma

Arguments

  • Infiltrat réticulé interstitiel du derme
  • Lymphocytes de petite taille (centrocytes) prédominent dans les lésions initiales, mélés à des centroblastes, plus abondant dans les lésions évoluées.
  • Follicules souvent mal individualisés
  • Immunohistochimie:
    • Marqueurs B
    • Bcl6 +, CD10+/-, Bcl2 -, CD5 -, CD43 -
  • Diag. differentiels:
    • Granulome annulaire
    • Dermite granulomateuse interstitielle
    • Si PN, dermatose neutrophilique
    • PNE: urticaire (PNN, Mastocytes)
    • Vascularite

Pronostic: >95% de survie à 5 ans. Grade et architecture sans influence sur le pronostic

 

  • Interstitial dermal infiltrate
  • Small lymphoctic infiltrate (centrocytes) mostly in early lesions, mixed with large, centroblasts, more abundant in advanced cases
  • Follicles often ill defined
  • Immunohistochemistry:
    • B-cell markers
    • Bcl6 +, CD10+/-, Bcl2 -, CD5 -, CD43 -
  • Diff. diagnosis:
    • Granuloma annulare
    • Intersticial granulomatous dermatitis
    • Neutrophilic dermatoses (if polymorphs are associated)
    • Urticaria (eosinophils)
    • Vasculitis

Prognosis: 5 y survival > 95%. Grade and growth pattern have no impact on prognosis.
