37 yo male, treated for myelodysplasia,
is hospitalised for fever, aplasia, and pneumonia. His condition worsened,
and developped respiratory distress, leading to endobronchic evacuation
of mucoid gelatinous casts, sent for pathology.
Homme de 37 ans connu et
trait pour syndrome myelodysplasique, hospitalis dans un contexte d'aplasie
fbrile et de pneumonie. Ses fonctions respiratoires se dtriorant, une
bronchoscopie est ralise pour vacuer des moules bronchiques glatineux.
 
Bronchus,
biopsy. Low power, marked inflammatory changes, with surface epithelial
erosion, and active congestive pattern of underlying connective tissue.
 
PAS stain,
discloses hyphae implanted on the surface epithelium, and invading the
connective tissue, with vascular extention.
 
Silver Grocott's
stain discloses the features of the mycelium.

Hyphae are
broad, septate and branch at about 45 degree.
