Case 010096
Patterns of the mucosa surrounding
the carcinomatous area.

Panoramic view of the
region oriented toward the grossly depressed area. Note the inflammatory
changes with atrophy. The sequence of the views below are representative
of the mucosa right, middle, and left areas .

The epithelial lining
discloses atypical foveolar hyperplasia.

Further, there is an
increase of the atypia, with some features suggestive on intra-epithelial
SRC.
Foveolar dysplasia in
this area shows rupture of the basement membrane with invasion of
the lamina propria.

Close-up on the previous
fields.
Although, these features
suggest in their presentation, SRC carcinoma in situ, the denomination
of carcinoma in situ is by consensus avoided in gastric lesions.
Severe dysplasia is the appropriate wording for such lesions. In
this case, it is severe or high grade foveolar dysplasia(Fenoglio-Preiser
in Gastrointestinal Pathology 2nd Ed.1999 p243).
