  Cas adressé pour avis, avec le diagnostic de lymphome cutané de type NK (CD56+), chez un sujet de 75 ans, pour un nodule cutané du cuir chevelu. Antécédent de carcinome épidermoïde du cuir chevelu de type acantholytique réséqué 2 ans auparavant.   Case submitted for 2nd opinion with the diagnosis of cutaneous T cell lymphoma NK type (CD56+), in a 75 yo patient complaining of a nodule in the occipital area. Previously, 2 years ago, an acantholytic squamous cell carcinoma of frontal area was resected.

Neuroendocrine carcinoma (Merkel cell carcinoma)

 
  Arguments:      
 
  • Localisation prédominante (50%)tête et cou, sous la forme d'un nodule érythémateux solitaire. Lésion du sujet agé.
  • Infiltration dermique sans epidermotropisme
  • Infiltration en nappes, profonde avec dissection des fibres de collagène
  • Cellules indifferenciées basaloides, noyaux à chromatine fine, poussièreuse
  • Nombreuses figures de mitoses
  • Rarement epidermotropisme pagetoide focal ou extensif.
  • Immunohistochimie:
  • Diagnostics differentiels:
    • Metastase d'un carcinome anaplasique du poumon,TTF1+, NF-
    • Lymphome malin
    • Sarcome d'Ewing
  • Pronostic:
    • Tumeur aggressive mais taille inférieure à 1 cm: bon pronostic
    • Metastase régional fréquente: necessite resection large et curage
    • 30 à 65% de survie à 5 ans

 

 
  • 50% of patients present with a solitary erythematous nodule of the face ans neck. Patients are often elderly.
  • Superficial dermal infiltration with a grenz zone
  • Extension in sheets, deep in the dermis dissecting the collagenous fibers
  • Undifferenciated basaloid cells, scanty thin acidophilic cytoplasmic rim. Nucleii round and vesicular with typically fine granular (dusty) chromatin
  • Numerous mitotic figures
  • Rarely focal or extensive intradermal pagetoid spread
  • Immunohistochemistry:
  • DIfferential diagnosis:
    • Metastasis of an oat cell carcinoma of the lung, TTF1+, NF-
    • Malignant lymphoma
    • Ewing's sarcoma
  • Prognosis:
    • Aggressive tumour, but size less than 1 cm: good prognosis
    • Regional nodal involvement common: wide excision with lymph node dissection recommended.
    • Range 30 to 65 % over-all 5 year survival
